Telecommunications & Networking Service Request (TSR)

Purpose:

The Telecommunications & Networking Services (TNS) TSR form is required to (1) request a new service, (2) change to an existing service, or (3) cancel a service from TNS.  This form provides TNS with the authorization to proceed with the service request on the customer’s behalf.   In addition, the TSR provides the approval to direct bill for the one-time and recurring charges until the service is removed or discontinued.

General Information:
  • Both recurring and non-recurring costs shown on this form are estimated amounts for the installation or service. For assistance with cost estimates, call the appropriate TNS service management representative.

  • Amendments may be made to this requisition only if the "Yes" box is checked. No exceptions will be made.
  • A detailed billing statement listing all applicable charges will be sent for information purposes following completion of the required service for non-recurring costs, or monthly for recurring costs. No IDCC is required since charges will be direct billed.

Obtain required approvals then forward the original. Send the completed form to the appropriate TNS Service Management representative at Telecommunications & Networking Services, University Support Building 2.
